Personalised care plans

Chart showing how we form a care plan for each guest We understand that everybody’s care needs are different, so we develop a tailored care package for each disabled guest who comes through our doors. We ask some basic medical questions at the point of booking and request that your GP fills in a medical certificate stating your condition prior to your break. One of our nurses then phones you to discuss your needs and your personal care plan is confirmed on arrival at the centre.

Expert nurse-led care

At Revitalise, you can rest assured that you will be in safe hands. We pride ourselves on delivering breaks that go far beyond traditional respite, but amidst all the fun that is on offer, we are committed to providing the best standards of care available.

We offer 24 hour on-call nursing and personal care, with fully qualified staff available on request all day, every day. We also have volunteers who provide support and companionship, both at the centre and out on excursions to make sure every guest has a truly fulfilling break.

Although one-to-one nursing care is not offered as standard, bespoke packages may be available at an additional cost for those with more complex needs.

If you have any queries about the care we provide, please call 0303 303 0145. You are also welcome to see what we do for yourself before booking your break, so please get in touch if you would like to arrange a visit to one of our centres.

  • If you use a hoist to transfer, you need to be aware that we can only use our slings as these are inspected every 6 months and certified by LOLER (Lifting Operations and Lifting Equipment Regulations). If you have a specialist sling you wish to use, you will need to bring an up to date  LOLER certificate with you. We cannot guarantee scheduled daily routines for guests that are accustomed to this level of assistance at home. However, we will try our best to ensure your needs are met as much as possible. Whilst out on trips it is not always possible to provide hoisting assistance when visiting the toilet. A number of our guests choose to use pads. Each guest participating on trips is assessed in advance.
  • If you choose to self-medicate you will be allocated a bedroom and key. A risk assessment will be conducted upon arrival to ensure your safety and that of other guests.
  • If you use personalised equipment at home such as a wheelchair or Zimmer frame, we would kindly request that you bring this along.
