Disclaimer
It is your responsibility to book directly with the holiday provider and/or the holiday home
provider at your own risk, having satisfied yourself about the suitability of all arrangements
including care and facilities where relevant. Revitalise Charity takes no responsibility and
accepts no risk or liability for any contractual relationship entered between you and the
provider. Revitalise Charity will not enter or become a party to any disputes arising between you and the provider. It is your responsibility to arrange adequate insurance cover for your holiday.
Terms and conditions
- Applicants (the disabled person and / or their family carer) are over 18 and applying for a grant for either a holiday, respite care, a trip, or an opportunity for social connection.
- The individual applicant is responsible for providing true and accurate information in their application. Provision of false information may result in the grant being refused or in Revitalise Charity requiring repayment of the grant.
- Revitalise Charity must receive evidence of the proposed holiday at the time of the application and a receipted payment for the holiday/travel being funded prior to a grant being paid to an individual. If the grant is being paid directly to a holiday provider we are happy to pay the final portion of a holiday on behalf of the individual grant recipient, subject to the maximum amount of grant funding we decide to award. Such payment is made on behalf of the individual and at their risk. All grants awarded by Revitalise Charity are paid by bank transfer.
- That payments by Revitalise Charity cannot be transferred to a different person or holiday venue or date from those stated on the application form.
- That Revitalise Charity grants are returned in their entirety if the proposed holiday is not taken for any reason what-so-ever. It is the applicant’s responsibility to notify Revitalise Charity if the holiday does not go ahead and to take suitable insurance to cover this risk and to select a suitable holiday.
- The grant can only be used for the reasons stated in the application.
- That Revitalise Charity reserves the right to contact the professional individual supporting the application, to make enquiries only about this application.
- That Revitalise Charity reserves the right to contact the individual applicant at any time via their preferred method of communication to discuss the application.
- That the individual returns a signed acceptance form if their application is successful.
- That Revitalise Charity receives written feedback about the holiday in electronic on paper form, and how it benefited the applicant and electronic photographs. Revitalise Charity may use this feedback and photos for promotional purposes, which may include the publication of this feedback to assist with fundraising and on their website. Your report and electronic pictures about how your holiday benefited you will also be passed to the holiday property provider for them to use on their website for promotional or PR purposes. Please contact the Charity in writing at the time of notification that you grant has been awarded if you have any concerns with this clause, so that these can be discussed. Such concerns must be notified when accepting your grant. Revitalise Charity will work with an individual to meet any issues with regards to any reasonable concerns.
